Rather than supplying growth hormone directly, tesamorelin binds the GHRH receptor on pituitary somatotrophs and stimulates the body's own pulsatile GH release. Because the pituitary remains subject to normal somatostatin feedback, the resulting GH profile is closer to physiological secretion than exogenous recombinant GH produces.

Tesamorelin is a stabilised 44-amino-acid analogue of human growth hormone releasing hormone (GHRH 1-44), modified with a trans-3-hexenoyl group at the N-terminus. That acylation protects the molecule from rapid degradation by dipeptidyl peptidase-4, which is what limits the usable half-life of unmodified GHRH.
